What it is

Blood pressure that swings suddenly and irregularly, sometimes surging high, sometimes dropping low.

Why it matters

It is a recognized feature of post-COVID dysautonomia and is often missed by a single office reading.

Normally a fast reflex, the baroreflex, senses your blood pressure and corrects small drifts within seconds, holding it inside a narrow range. After COVID this buffer can fail, so the same pressure that used to stay level now swings, surging above the range and dropping below it. The swings are the failure of the control system, not a sign that you are doing something wrong.

Large blood-pressure variability, including paroxysmal hypertension, sometimes with hypertension alternating with hypotension; among the more disabling features of post-viral dysautonomia.

Mechanism

Driven by autonomic dysfunction and baroreflex failure, with contributions from adrenergic and angiotensin-receptor autoantibodies and a vasoconstrictive ACE2 to angiotensin II shift.
