Insomnia / sleep disruption

Sleep often breaks in long COVID, and not in one single way. Some people develop classic insomnia, others lose the regular timing of sleep, and still others swing toward excessive sleepiness or new sleep disorders. The common thread is that sleep, which the body needs to recover, becomes unreliable.

Disrupted sleep is one of the most common long COVID complaints, and it is not a single problem. A sleep clinic evaluation of long COVID patients found several distinct syndromes, from chronic insomnia to excessive daytime sleepiness, rather than one uniform pattern.

The range of sleep problems

In the clinical evaluation, long COVID patients showed obstructive sleep apnea, chronic insomnia, primary excessive sleepiness, REM sleep behavior disorder, and new-onset circadian phase delay, where the body clock shifts later. Some met criteria for idiopathic hypersomnia or narcolepsy-type sleepiness.

This range is striking. Long COVID does not just cause trouble falling asleep; it can disturb the architecture, timing, and regulation of sleep in multiple ways. That diversity means a single label like insomnia undersells what can go wrong, and that different patients need different evaluations. Obstructive sleep apnea in particular is worth catching, because it is common, treatable, and produces fatigue and brain fog that can be mistaken for long COVID itself.

Insomnia and lost regularity

The most familiar pattern is insomnia: difficulty falling or staying asleep despite the chance to. But the study highlighted something subtler and important, a persistent loss of sleep regularity, where the timing of sleep and wake drifts and varies from night to night.

Regularity matters as much as duration. The body clock relies on consistent timing of sleep, light, and activity, and losing that consistency can worsen fatigue and cognition even if total sleep time looks adequate. This connects long COVID sleep problems to the broader disruption of daily rhythms. Two people sleeping the same number of hours can fare very differently if one keeps a steady schedule and the other sleeps at random times, which is why regularity has become a focus in its own right.

Why sleep disruption feeds the illness

Sleep is not passive rest; it is when the body clears metabolic waste, consolidates memory, and regulates immune and hormonal systems. When sleep is fragmented or mistimed, these restorative processes suffer, which can amplify the inflammation, cognitive symptoms, and exhaustion of long COVID.

This creates a vicious loop: the illness disturbs sleep, and poor sleep worsens the illness. It also links to the low cortisol and disrupted rhythm findings elsewhere, since the systems that govern sleep and daily hormonal rhythms overlap. Improving sleep is one place where the loop might be interrupted.

Supporting sleep regularity, consistent sleep and wake times, light exposure in the day, and a steady routine, is a reasonable foundation that respects the body clock. For persistent or severe problems, a formal sleep evaluation can identify a specific, treatable disorder.

How to read claims about it

Be wary of treating all long COVID sleep problems as ordinary insomnia to be solved with a sleeping pill. The evidence shows several distinct disorders, some of which need specific diagnosis and treatment rather than a sedative.

Hold long COVID sleep disruption as a real, varied, and consequential part of the illness that deserves proper evaluation. Improving it is worthwhile in its own right and may ease other symptoms, given how central sleep is to recovery.
