SIM01 synbiotic

The first randomized trial to show that fixing the gut can ease long COVID. SIM01, a defined mix of beneficial bacteria and the fibers that feed them, improved digestion, fatigue, and concentration against placebo over six months.

Short version: SIM01, a specific synbiotic, beat placebo in a randomized trial for several long COVID symptoms. A real proof of concept that gut-targeted treatment can help, though benefit was partial.

Rebuilding the gut community

SIM01 is a synbiotic, three Bifidobacterium strains plus prebiotic fibers, designed to restore the helpful bacteria depleted in gut dysbiosis after COVID.1

A healthy gut wall is sealed and fed by bacteria that make butyrate. After COVID those bacteria can stay depleted while the barrier loosens. Microbial fragments then leak into the blood and keep the immune system on alert. This gut-to-body loop is one proposed route from the intestine to fatigue, brain fog, and disturbed signaling along the vagus nerve.

The strongest gut evidence so far

In a triple-blind randomized placebo-controlled trial of 463 patients over six months, SIM01 improved several long COVID symptoms, with more recipients reporting better digestion, fatigue, concentration, and memory than controls, and a low rate of side effects.1

How to read it

This is the first RCT showing a gut-targeted treatment can ease long COVID symptoms, a meaningful proof of concept. It is a specific formulation studied in one population; results may not transfer to over-the-counter probiotics, and benefit was partial.

What we don't know

Honest about the edges of the evidence. These are open questions, not settled answers.

  • Whether the benefit replicates in other populations.
  • How it compares with cheaper probiotics or fiber.
  • How long treatment should last.
  • Which symptoms respond most.
